In a minimal gnome-shell installation, the Extensions app refuses to start due to missing dependency:

$ gnome-extensions-app

(org.gnome.Extensions:927): Gjs-CRITICAL **: 12:50:39.747: JS ERROR: Error: Requiring Adw, version 1: Typelib file for namespace 'Adw', version '1' not found


Installing libadwaita solves the problem. Please add it to depends=().
