I don't now what exact package causes problem, but I see the icon in all DE except KDE.

FWIW, I'm seeing the same thing. No icon in the launcher (black square) and no icon in the panel when it's running (green paper). Fully up to date system, testing enabled, plasma-desktop 5.4.1-2

Actually, the breeze icon for virtualbox does look like a black square unless you look closely:

Is that what you're seeing?

About the panel: does disabling "Use launcher icon for running applications" in taskbar settings fix it?

That is what I'm seeing in the launcher. Working as intended there, it seems.

Yes, disabling that option brings back the normal icon in the taskbar. So the only issue that I'm seeing seems to be that that breeze icon isn't showing in the taskbar. Maybe yurikoles is seeing something different.

I never really cared, so I never looked into this earlier. Just thought I could help move this one along.

That's https://bugs.kde.org/show_bug.cgi?id=351624, not related to virtualbox (other applications like kmail or dragon are also affected) and already fixed upstream for Plasma 5.5

In any case this can't be the OP's issue, since it was introduced in Plasma 5.4 and this report is from June.
