The comment you make is not true as 'kernel (hd?,?)/memtest86+/memtest.bin' can be correct if /boot is a different partition. You just have to adapt this line to your own needs.

I still think it would be a good idea to illustrate where memtest86+ is actually installed.

memtest is always installed in /boot, but if your /boot is a separate partition, the entry that grub needs cannot have /boot in it.
