Currently, I cannot use mutt for company email where we use exchange (unfortunately), so this would be very much welcome. In an ideal world, this should be upstreamed and built in by default, but as the years show, we are not there...
This task depends upon

I have no idea what you wish to see done to the mutt package.
Shall I guess from digging up that old forum link? From googling NTLM?

Anyhow, you say "this should be upstreamed" and, whatever "this" is, yes, it really should be upstreamed.
Arch Linux is not Debian: we do not have the manpower to virtually fork every upstream project by maintaining series of patches to them; in fact, we believe this is detrimental to the OSS ecosystem as a whole.

I suggest you either:
- write up a mutt-ntlm package and maintain it on the AUR;
- fork upstream mutt, integrate the ntlm patch, and maintain the resulting piece of software.
