Enabling memberof overlay on frontend database (bdb) causes slapd to SEGFAULT when adding new user and group objects to the LDAP tree. See OpenLDAP issue tracker for more detailed info.
Applying the attached patch (modified for 2.4.38 source tree) resolves this issue for me.

1) Enable memberof overlay.
2) Populate LDAP tree with user and group data.
