Details

The icon of KVIrc has a resolution that is too small for the window switcher (that one you get when pressing ALT-Tab) of xfce (not sure which version number to provide: xfce4-session 4.10.0-3).
Or it could be the issue that the right icon is placed at the wrong place.

It is upstream issue I think.

Use

xprop | grep 'NET_WM_ICON(CARDINAL)'

and point window to get icon sizes.

For kvirc it is
_NET_WM_ICON(CARDINAL) = Icon (16 x 16):
