I've a Toshiba laptoop with an nvidia GeForce FX Go5200 32M video card and I've the same problem.
The issue happens when I switch from my external monitor to the laptop one or viceverca using xrandr.
Otherwise it does not happen.

Moreover I've discovered that killing X with Ctrl-Alt-Bksp when the problem happens, will resolve it till the following use of xrandr.

PS I use xrandr to switch between the internal (1280x800) and external monitor (1360x768) turning off the monitor I'm not using. I don't know if the fact that the two monitors have different resolutions may be one of the causes of the problem.
