I don't believe gnome-shell 3.2 needs to be restarted for extensions to work. Although, none the less it doesn't work for me either. Hitting the switch ON/OFF effectively does nothing.

Anyway, here's what looking glass says:
error t=2011-10-02T13:28:022 Extension "alternative-status-menu@gnome-shell-extensions.gnome.org" Had
error TypeError: statusMenu is undefined

For me the patch does not fix the problem.
