This forum thread is pretty clear on what is wrong, and how I solved it.

Steps to reproduce: Install nvidia and nvidia-utils from repos. Workthe DirectX clients.

Steps to remedy: Carefully remove the nvidia and nvidia-utils using pacman, and any dependent lib32 libraries. Install the NVidia drivers from the NVidia website, and normal operation is regained.

All I can think of is there some individual libary that may be missing, or that the kernel module wants to be compiled freshly on each new kernel even within the same 'series'. But I cannot confirm this.

it's NOT recommended to install nvidia by hand, use the nvidia-beta or other pkg from AUR or ABS

With kernel 2.6.38, and NVidia 270.41.03-1 this bug is gone.

I understand that installing a package manually from 3rd parties is frowned upon. However, it was the only mechanism I was aware of to remedy my problem. The pacman -Syu updates on April 11th, 2011 of both the NVIdia from pacman resolved the issue.

I took care to surgically remove the old factory manually installed NVIdia drivers and all it's lib32 components and standard (x65 for me) components.

I do know, the old issue was a libGL related issue where the older driver in pacman wasn't updating this particular file.

As of today, to be sure, I "pacman -Syf" the nvidia packages and all is well.
